Full-mouth functional and aesthetic rehabilitation of a patient with severe mandibular atrophy: A multidisciplinary case report

Özet
Objective: Severe alveolar ridge atrophy complicates implant placement and often requires combined surgical, periodontal, and prosthetic interventions to
achieve predictable, long‑term functional and aesthetic outcomes.
Aim To present the multidisciplinary management of a patient with a severely atrophic mandible using synchronized surgical and prosthetic protocols.
Materials & Methods: A completely edentulous mandible with knife‑edge ridge morphology was treated with alveoloplasty to improve ridge contour,
followed by placement of three implants (Ankylos®, Dentsply Sirona; Ø 3.5 mm, 11 mm). Buccal defects were augmented using guided bone regeneration
with a xenogenic graft (Geistlich Bio‑Oss®) and a resorbable collagen membrane (Geistlich Bio‑Gide®). Two months later, the maxillary phase included 2
mm crown lengthening, apical resection of tooth #23, and insertion of two implants in the #24–26 region. Provisional prostheses maintained function and
aesthetics throughout healing. After 5 months, definitive rehabilitation was completed with a fixed maxillary bridge and a mandibular implant‑supported
overdenture.
Results: All implants achieved successful osseointegration. Radiographic and clinical examinations confirmed stable peri‑implant bone in the regenerated
areas, with no biological or mechanical complications during follow‑up. Functional and aesthetic outcomes were highly satisfactory.
Conclusion: In severe mandibular atrophy, combining alveoloplasty, GBR, and carefully coordinated prosthetic planning enables predictable implant
placement and stable, functional, and aesthetic full‑mouth rehabilitation.
