Pediatric Cataracts: Our Surgical Approach

Özet

Purpose: Retrospective evaluation of the clinical characteristics of the pediatric cataract. patients, our surgical approach and the surgical outcomes in our clinic and to outline the current approach on pediatric cataracts. Method: The records of patients who underwent pediatric cataract operation between May 1993 and April 2004 in Beyoglu Eye Research and Training Hospital, and who were followed-up regulary, were retrospectively examined. The cases were evaluated with respect to age of surgery, concomitant ocular and systemic abnormalities, operation technique, complications, resultant visual acuities and refractive errors, and optical rehabilitation modalities. Results: A total of 77 eyes of 47 patients (17 unilateral, 30 bilateral) were operated. The mean age of surgery was 6.27 +/- 4.59 years, duration of follow-up was 42.16 +/- 18.24 months. Best corrected visual acuity was evaluated in three separate age groups and significant improvement was detected ( p<0.05). Inflammatory membrane formation was detected in the early postoperative period. For the optical rehabilitation, primary intraocular lens (IOL) implantation was performed in 32 eyes; aphakic spectacles or contact lenses were used and secondary IOL implantation was planned in 15 eyes. Conclusion: In pediatric cataract surgery, prognosis depends on the correct timing of the intervention, proper surgical technique selection, close follow-up and management for the treatment of amblyopia and gain of binocular vision. Best postoperative visual results in both unilateral and bilateral congenital cataract cases could be gained by early cataract extraction, posterior capsulotomy, anterior vitrectomy and foldable IOL implantation.
