THE IMPACT OF DISEASE MANAGEMENT PLATFORM (DMP) IMPLEMENTATION ON GLYCEMIC CONTROL OF TYPE 2 DIABETES MELLITUS IN PRIMARY CARE

Abstract

This retrospective case-control study investigated the impact of the Disease

Management Platform (DMP/HYP), used in family medicine practice, on the glycemic

control of patients diagnosed with Type 2 Diabetes Mellitus. Data from 160 patients

registered at Izmir Buca Family Health Center No. 7 were analyzed; the mean age of

participants was 60.5±9.9 years, mean body mass index (BMI) was 30.8±12.4 kg/m2,

and overall mean HbA1c was 8.83±1.32. The study found that patients followed within

the DMP framework had a significantly lower mean HbA1c (7.72±0.73) compared to

those not followed (9.98±1.41) (p<0.001). While the rate of reaching the glycemic

target (HbA1c <7) was 61.7% in the DMP group, this rate remained at 27.8% in the

non-DMP group (p<0.001). In multivariate logistic regression analysis, after controlling

for age, gender, and BMI, DMP use was identified as an independent predictor that

increased the probability of having controlled diabetes by 4.5 times (OR: 4.5; 95% CI:

2.3–9.1; p<0.001). These findings demonstrate that the use of digital monitoring and

clinical decision support systems in primary care significantly improves diabetes

management success and patient outcomes.
