Perinatal Transmission of Hepatitis B Virus despite the Active and Passive Immunoprophylaxis: A Case Report

Özet

One of the main transmission routes of Hepatitis B Virus (HBV) is the infection of newborn from an infected mother (perinatal [ vertical] transmission). Perinatal transmission can occur in almost 5% of the infants born from anti-HCV positive mothers. HBsAg positivity was determined at the fourth month of the pregnancy in a twenty-two year old mother in a different center. The infant was delivered by elective cesarean section without any complication. HBV vaccine and HBIG were administered to the newborn within the first hour life. The HBV vaccination series were completed with the administration of second and third doses during one month and 6 months of age. HBsAg positivity was determined when the infant was 7.5 months-old. The laboratory results of the mother were as follows (i) HBsAg: positive, anti-HBs: negative, HBeAg: positive, anti-HBe: negative, anti-HBc total: positive, anti-HBc IgM: negative, HBV DNA PCR: 2x10(8) IU/mL and anti-HCV: positive. The mother's HCV RNA was negative by real time-PCR. In the laboratory analysis of the infant revealed that HBsAg was positive, anti-HBs was negative, HBeAg was positive, anti-HBe was negative, anti-HBc total was positive, anti-HBc IgM was negative, HBV DNA PCR was 5x10(8) IU/mL and anti-HCV was negative. Although administration of HBIG with Hepatitis B vaccine is the most effective way of preventing the perinatal transmission, HBV infection may occur in 5% - 10% of cases. In our case, perinatal transmission of HBV occurred. To decrease the rate of perinatal transmission, an oral antiviral agent may be administered during the third trimester of the pregnancy in order to reduce the level of maternal viremia.
