Abstract
Objective: The study aims to compare the clinical and radiographic outcomes of peri-implantitis patients who smoke and those who do not, and to evaluate the inflammatory effects of smoking on peri-implant tissues using the MMP-8 test.
Materials and Methods: The study was conducted on 10 implants, including 5 smokers and 5 non-smokers, each with at least one dental implant (8 females and 2 males, average age ± SD; 49.7±16.09). Demographic data of smoker and non-smoker patients, including age, gender, systemic disease status, medication use, smoking habits, education level, monthly income, and tooth-brushing habits, were recorded separately. Clinical peri-implant and radiographic parameters, such as peri-implant bone loss and the type of destruction in peri-implant tissues, were documented.
Results: The frequency of tooth brushing was recorded as 1.6 ± 0.49 times per day in non-smokers and 1.3 ± 0.86 times per day in smokers. The mean time for implant placement and loading was 8± 4.05 years in non-smokers and 10.4 ± 5.46 years in smokers. The peri-implant bone loss rate was measured at 27.6% ± 23.16% in non-smokers and 15.6% ± 12.22% in smokers. Gingival Crevicular Fluid (GCF) values around implants were 98.8 ± 61.8 in non-smokers and 51.4 ± 37.83 in smokers. In the control tooth, GCF was 93.5 ± 60.12 in non-smokers and 75.7 ± 69.28 in smokers. The average probing depth (PD) was 2.08 ± 0.42 in non-smokers and 2.42 ± 0.42 in smokers. The average clinical attachment level (CAL) was -2.76 ± 1.28 in non-smokers and -3.28 ± 1.69 in smokers. The average bleeding on probing (BoP) was calculated as 51.2% ± 14.72% in non-smokers and 48% ± 5.10% in smokers. The mean plaque index (PI) was 44.2% ± 8.75% in non-smokers and 55.2% ± 8.35% in smokers. The MMP-8 test, applied to assess the inflammatory load, showed negative results in all cases
