Review of 1127 transplant kidney biopsies performed over a ten year period for non or failing function revealed 60 with clear-cut polymorphonuclear leucocyte (PML) infiltration. These could be divided into three broad sub-groups. Twenty-six (43%) were associated with acute rejection and 10 patients in this group also had concomitant culture-positive urinary tract infections. Thirteen patients (22%) had severe PML infiltration but in all except two cases the patients had documented urinary tract infections or were on antibiotics at the time of the biopsy. Twenty-one patients (35%) had mild PML infiltration. However, of the nine patients in this group not on antibiotics at the time of biopsy only one had evidence of urinary tract infection suggesting that the PML infiltrate in these patients may represent early rejection rather than infection.
