Üriner sistem enfeksiyonu ile yatarak takip edilen hastalarda izole edilen E.coli ve K.pneumoniae suşlarının fosfomisin ve diğer antibiyotiklere olan duyarlılığı

Özet
GİRİŞ: Üriner sistem enfeksiyonu (ÜSE) erişkinlerde en sık görülen bakteriyel enfeksiyonlardan biridir. Escherichia coli, en sık izole edilen mikroorganizma olup sıklıkla toplum kökenli enfeksiyonlarda etken olarak gözlenirken, hastane kökenli olanlarda Klebsiella, Proteus, Pseudomonas, Enterobacter türleri, Enterokoklar ve Stafilokoklar da etken olabilmektedir. Tedavide uygunsuz antibiyotik kullanımına bağlı olarak antibiyotik direnci artmakta bu da tedavi yanıtsızlığına ve ciddi enfeksiyonlarda mortalite artışına neden olmaktadır. Uygun ampirik antibiyotik başlanması için bölgesel direnç oranlarının bilinmesi önem taşımaktadır. Bu çalışmada kliniğimizde ÜSE tanısı ile yatarak takip edilen olguların idrar kültüründe en sık izole edilen mikroorganizmalar olan E.coli ve K.pneumoniae suşlarının antibiyotik duyarlılıklarının incelenerek en uygun tedavinin düzenlenmesine ve direnç gelişiminin önlenmesine katkı sağlamak amaçlanmıştır. GEREÇ-YÖNTEM: Bu çalışmada 01.11.2016-01.11.2019 tarihleri arasında ÜSE tanısı ile kliniğimizde yatarak takip edilen hastaların idrar kültürlerinde izole edilen E.coli ve K.pneumoniae suşlarının in vitro antimikrobiyal duyarlılıklarının sonuçları retrospektif olarak incelenmiştir. Laboratuvarımıza gönderilen idrar örneklerinin önce hücre sayımları, sonra konvansiyonel yöntemlerle kültürleri yapılmıştır. Üretilen suşların identifikasyonu ve antimikrobiyal duyarlılık testleri için BD Phoenix 100 otomatize sistemi (Becton-Dickinson, ABD) kullanılmıştır. BULGULAR: Kliniğimizde yatarak takip edilen ve idrar kültüründe E. coli üreyen 56 hasta, K. Pneumonia üreyen 50 hasta olmak üzere toplamda 106 kültür sonucu incelendi. Hastaların yaş ortalaması 60,8±18,7 yıl olup, 56’sı (%52,8) kadındı. İki etkene ortak bakıldığında genişlemiş spektrumlu beta-laktamaz (GSBL) oranı %68.8 (34 olgu E. coli, 39 olgu K. pneumoniae) idi. E. coli için fosfomisin, meropenem ve ertapeneme direnç tespit edilmedi. Seftriakson, trimetoprim-sulfametoksazol ve siprofloksasin direnci ise sırasıyla %85,8, %60,7 ve %73,3 bulundu. Fosfomisin, meropenem ve ertapenem direnci K.pneumoniae için sırasıyla %42, %2 ve %18 oranında tespit edildi. Seftriakson, trimetoprim-sulfametoksazol ve siprofloksasin direnci ise sırasıyla %94, %50 ve %72 bulundu. İki etkende de amikasine direnç tespit edilmezken, E. Coli için %21,5, K. pneumoniae için ise %8 oranında orta duyarlılık saptanmıştır. Diğer antibiyotik direnç oranları Tablo 1’de verilmiştir. SONUÇ: Ülkemizde ve diğer ülkelerde yapılan çalışmalarda GSBL (+)’liği ve karbapenem direncinin giderek arttığı görülmektedir. GSBL (+) etkenlerin neden olduğu ÜSE’lerde karbapenemler kullanılmaktadır. Karbapenemlere de dirençli suşlarda tedavi seçenekleri azalmaktadır. Hastanelerde bu suşların hızlıca identifikasyonu ve bu hastaların hızlıca izole edilmesi etkenin yayılımını önlemek açısından önemlidir. Üriner sistem enfeksiyonlarında ampirik tedavi için en sık etken olan bakterilerin duyarlılıklarının bilinmesi gerekmektedir. Toplamda 34 (%60,7) GSBL (+) E. Coli, 39 (%78) GSBL (+) K. Pneumoniae suşu tespit edilmiştir. Bu oranlar yıllar içinde artan antibiyotik direnci sonuçlarını desteklemektedir. Yüksek direnç oranı nedeniyle siprofloksasin ve trimetoprim sülfametoksazol yerine ayaktan ampirik tedavide ilk seçenek olarak fosfomisin kullanımın tercih edilmesi gerektiğini değerlendirmekteyiz.
INTRODUCTION: Urinary tract infection (UTI) is one of the most common bacterial infections in adults. While Escherichia coli is the most frequently isolated microorganism, it is frequently observed as a causative agent in community-acquired infections, whereas Klebsiella, Proteus, Pseudomonas, Enterobacter spp., Enterococci and Staphylococci may be the causative agents of nosocomial infections. Antibiotic resistance increases due to inappropriate use of antibiotics in treatment, resulting in treatment failure and increased mortality in severe infections. It is important to know the regional resistance rates in order to initiate appropriate empirical antibiotics. The aim of this study was to investigate the antibiotic susceptibility of E.coli and K.pneumoniae strains, which are the most frequently isolated microorganisms in urine culture, and to contribute to the regulation of the most appropriate treatment and prevention of resistance development. MATERIALS-METHODS: In this study, in vitro antimicrobial susceptibility of E.coli and K.pneumoniae strains isolated in urine cultures of patients hospitalized in our clinic with the diagnosis of UTI between 01.11.2016-01.11.2019 were investigated retrospectively. Urine samples sent to our laboratory were first cell counted and then cultured by conventional methods. BD Phoenix 100 automated system (Becton-Dickinson, USA) was used for identification and antimicrobial susceptibility tests of the strains produced. RESULTS: A total of 106 cultures including 56 patients with E.coli and 50 patients with K.Pneumonia were hospitalized in our clinic. The mean age of the patients was 60,8 ± 18,7 years and 56 (52,8%) were female. Extended spectrum beta-lactamase (ESBL) ratio was 68,8% (34 cases were E.coli, 39 cases were K.pneumoniae). No resistance to phosphomycin, meropenem and ertapenem was detected for E.coli. Resistance to ceftriaxone, trimethoprim-sulfamethoxazole and ciprofloxacin were 85,8%, 60,7% and 73,3% respectively. Phosphomycin, meropenem and ertapenem resistance were 42%, 2% and 18% for K.pneumoniae, respectively.. Ceftriaxone, trimethoprim-sulfamethoxazole and ciprofloxacin resistance were 94%, 50% and 72% respectively. Amikacin resistance was not detected in either of the two agents, while intermediate sensitivity was found to be 21.5% for E.coli and 8% for K.pneumoniae. Other antibiotic resistance rates are given in Table 1. CONCLUSION: In our country and in other countries, it is seen that ESBL (+) and carbapenem resistance increase gradually. Carbapenems are used in UTIs caused by ESBL (+) agents. Treatment options for carbapenems resistant strains are decreasing. The rapid identification of these strains in hospitals and the rapid isolation of these strains is important to prevent the spread of the causative agent. The susceptibility of bacteria, which are the most common agents for empirical treatment of urinary tract infections, needs to be known. A total of 34 (60.7%) ESBL (+) E.Coli and 39 (78%) ESBL (+) K.Pneumoniae strains were detected. These rates support the increasing antibiotic resistance results over the years. We consider that phosphomycin should be preferred as the first choice in outpatient empirical treatment instead of ciprofloxacin and trimethoprim sulfamethoxazole due to its high resistance rate.
