Left atrial appendage (LAA) aneurysms are extremely rare. We present 68-year-old female suffered from giant aneurysm of the left atrial appendage and long standing history of atrial arrhythmias. We performed transesophageal echocardiogram that exposed a left atrial appendage aneurysm 9 x 6 x 3 cm in size and electrophysiologic study showing inducible nonsustained atrial foci of dysrhythmia arising from the left atrium, which quickly degenerated into atrial fibrillation. The case was successfully treated with resection of the aneurysm.
