Ekstraplevral Yerleşimli Retrosternal Hematomların Medikolegal Değerlendirme Süreci

Özet
Göğüs travmalarının %7,1'inde ekstraplevral kanama insidansı vardır. Ekstraplevral hematom, paryetal plevra ile endotorasik fasya arasındaki kanamadır. Bu alan, göğüs duvarını oluşturan yapılar arasında yer alır. Adli olgularda, göğüs boşluğunun bütünlüğünün bozulması/bozulmaması medikolegal değerlendirme kriterlerini ciddi olarak etkilemektedir. Bu nedenle retrosternal hematomların, göğüs boşluğu ile ilişkili olup olmadığının kesin olarak belirlenmesi, medikolegal değerlendirme açısından önemlidir. Trafik kazası sonrası 51 yaşındaki erkek olgumuzda, korpus sternide, sağda 2 ve 5, solda 2 ve 3. kostalarda fraktür tespit edildi. Retrosternal alanda 6×4×1,7 mm boyutlarında hematom ile uyumlu dansite artımı olduğu, göğüs boşluğu içinde hemotoraks/pnömotoraks ve akciğer kontüzyonu olmadığı saptandı ve hasta değerlendirildi. Doğru tanı koymak hak kayıplarının önlenmesinde önemli bir kriterdir. Bu çalışmada, retrosternal ekstraplevral hematomlu hastaların değerlendirme sürecinde dikkat edilmesi gereken noktalar paylaşılarak, adli tıp pratiğinde standardizasyon sağlanması için mevcut rehberde yapılabilecek güncelleme önerisi tartışılacaktır.
The incidence of extrapleural hemorrhage in chest traumas is approximately 7.1%. Extrapleural hematoma refers to bleeding between the parietal pleura and the endothoracic fascia. This compartment lies within the structures of the chest wall. Whether or not the integrity of the thoracic cavity is disrupted affects medicolegal assessment criteria. Therefore, determining their association with the thoracic cavity is crucial for medicolegal evaluations. A 51-year-old male, following a traffic accident, had fractures of the sternal corpus; 2nd/5th right ribs, and 2nd/3rd left ribs. A density consistent with a hematoma measuring 6×4×1.7 mm was observed in the retrosternal area. No hemothorax, pneumothorax, pulmonary contusion was detected within the thoracic cavity, and the patient evaluated. Accurate diagnosis is crucial to prevent loss of rights. This study highlights the key points to be considered during the evaluation of patients with retrosternal extrapleural hematoma, and discusses potential updates to the existing guideline for standardization.
