Fragment Reattachment of a Traumatic Complicated Crown Fracture by Partial Pulpotomy : A 20-Month Follow-Up Case Report

Abstract

Background: Crown fractures of permanent incisors are among the most common traumatic dental injuries in children, most frequently affecting the maxillary central incisors. Complicated crown fractures, involving enamel–dentin fractures with pulp exposure, require prompt management to preserve pulp vitality and maintain tooth function and aesthetics.

Case Report: A systemically healthy 10-year-old male patient visited to our clinic due to the complaint of fractured upper anterior tooth caused by a fall. The patient brought the fractured tooth fragment stored in water. According to the medical history obtained, the trauma had occurred approximately 3 hours prior to dental visit. Clinical and radiographic examinations revealed a complicated crown fracture involving the pulp tissue in the permanent maxillary left central incisor(#21), with no evidence of root fracture or periapical pathology. The tooth demonstrated sensitivity to thermal stimuli.

Treatment: Under local anesthesia, a partial pulpotomy was performed on tooth (#21) using Biodentine following the vital pulp therapy procedure, and the fractured fragment was subsequently reattached following an adhesive protocol.

Follow Up: At the 20-month follow-up examination, pulp vitality was maintained, and the tooth remained asymptomatic with positive responses to vitality testing. Radiographic findings showed no signs of periapical pathology.

Conclusion: In permanent teeth with complicated crown fractures, preservation of pulp vitality through partial pulpotomy combined with conservative fragment reattachment can provide predictable and successful clinical outcomes while supporting biological healing.
