Dişeti çekilmesi veya yumuşak doku çekilmesi,
dişeti kenarının dişin mine-sement birleşiminin apikaline yer değiştirmesi
olarak tanımlanır. Dişeti çekilmesi ile kök yüzeyinin açığa çıkması sıklıkla dentin
aşırı duyarlılığı, kök çürükleri, çürük olmayan servikal lezyonlar, zayıflamış
plak kontrolü ve estetik olmayan görünüm ile ilişkilidir. Başlıca dişeti
çekilmesini oluşturan faktörler; ince dişeti dokusu, mevcut olmayan veya sığ
keratinize dişeti bandı (<2 mm), mukogingival birleşimin ötesine uzanan
sondalama derinlikleri ve progresif dişeti çekilmesi ve/ veya hastanın inflamatuar
periodontal hastalık öyküsüne sahip olmasıdır. Anormal frenulum pozisyonu,
güçlü dudak kasları, travmatik ağız hijyeni alışkanlıkları, subgingival
restorasyon sınırları, sigara ve diyabet gibi sistemik durumlar da dişeti
çekilmelerinde etken olabilmektedir. Dişeti çekilmelerinin tedavisinde birçok
cerrahi yöntem mevcut olmasına rağmen bağ dokusu grefti (BDG) altın standart
olarak kabul edilmektedir. Ancak, BDG çoklu dişeti çekilmelerinin tedavisi için
sınırlı miktarda elde edilebilmekte, ince damak yapısına sahip kişilerde nekroz
veya dehisens gelişebilmekte, operasyon sonrası verici bölgede kanama ve ağrı
oluşabilmektedir. Günümüzde BDG’ ne
alternatif olarak otojen olmayan aselüler dermal matriks, mine matriks proteini
veya kollajen matriks kullanılabilir. Ayrıca, zengin büyüme faktörleri içermesi nedeniyle
son yıllarda otojen trombositten zengin fibrin (PRF) ürünleri tercih
edilmektedir.
Titanyum tüpte hazırlanan trombositten zengin fibrinde (T-PRF) kan, titanyum
ile aktive olmakta, PRF’ den daha matur ve organize bir fibrin yapısı
göstermektedir. Sistemik
olarak sağlıklı, 55 yaşında hasta diş hassasiyeti ve estetik problemler sebebi ile
kliniğe başvurdu. Klinik muayenesinde, travmatik fırçalama sonucu oluşan, servikal
çürük içermeyen 10 dişte çoklu dişeti çekilmeleri tespit edildi. 2017 yılında “American
Academy of Periodontology ve European Federation of Periodontology’’ tarafından
yapılan güncel sınıflamaya göre dişeti fenotip özellikleri ve dişeti
çekilmeleri değerlendirildi. Toplamda 10 dişeti çekilmesinde, 8 dişte Tip 1, 2
dişte Tip 2 dişeti çekilmesi mevcuttu. Çoklu dişeti çekilmeleri Web Koronale Pozisyonel
Flep Tekniği ve T-PRF ile iki seansta tedavi edildi. Operasyon sonrası
iyileşmeler sorunsuzdu. 3 yıl takip sonucunda çoklu dişeti çekilme defektlerinde
%85 oranında ortalama kök kapanması elde edildi. Çoklu dişeti çekilmelerinin
web koronale pozisyonel flep ve T-PRF ile tedavisinde uzun dönem başarılı
sonuçlar elde edilmiştir.
Gingival recession or soft tissue recession is
defined as the displacement of the gingival margin apical to the cemento-enamel
junction of a tooth. The exposure of the root surface by gingival recession is
frequently associated with dentine hypersensitivity, root caries, non-carious
cervical lesions, compromised plaque control and unaesthetic appearance. The
major susceptibility factors are thin gingival tissue, absence or a narrow band
(<2 mm) of keratinized tissue, probing depths extending beyond the mucogingival
junction and a positive history of progressive gingival recession and/or
inflammatory periodontal disease. Abnormal
frenulum position, strong lip muscles, traumatic oral hygiene habits,
subgingival restoration limits, smoking and systemic conditions like diabetes
can also be reasons in gingival recessions. Although there are many surgical methods in the treatment of gingival
recessions, connective tissue graft (CTG) is considered the gold standard. However, CTG can be obtained in limited amounts
for the treatment of multiple gingival recessions, necrosis or dehiscence may
develop in patient with thin palate structure, bleeding and pain may occur in
donor site after the operation. Currently, a non-autogenous acellular dermal
matrix, enamel matrix protein or collagen matrix can be used as an alternative
to CTG. In addition,
autogenous platelet-rich fibrin (PRF) products have been preferred in recent
years due to the including rich growth factors. In titanium prepeared
platelet-rich fibrin (T-PRF) prepared in a titanium tube, the blood is
activated by titanium and shows a more mature and organized fibrin structure
than PRF. Systemically healthy,
55-year-old patient referred to the clinic due to dentin hypersensitivity and
aesthetic problems. Clinical
examination, multiple gingival recessions without caries was detected in 10
teeth that occured as a result of traumatic brushing. Gingival phenotype features and gingival
recession were evaluated according to the current classification made by the
"American Academy of Periodontology and European Federation of
Periodontology" in 2017.Total of 10 gingival recessions from 8 teeth were
classified as R1 and 2 teeth were classified as R2. The all multiple gingival
recessions were treated with Web Coronally Positional Flap Technique and T-PRF
in two sessions. Post-operative recovery was uneventful. As a result of 3 year follow-up percentage of
85% root coverage was obtained in the multiple gingival recession defects. Long-term successful results
have been obtained in the treatment of multiple gingival recessions with web
coronal positional flap and T-PRF.
