GİRİŞ-AMAÇ:
Over kanseri; ikinci en sık görülen jinekolojik malignite ve en sık ölüme sebep
olan jinekolojik malignitedir. Yüksek dereceli seröz over kanseri %70 oranla en
sık görülen ve daha kötü klinik sonuçları olan tiptir. Geç tanı; %75 oranında
(InternationalFederation of Gynecology and Obstetrics- FIGO III-IV) ve
kemoterapiye gelişen direnç sebebiyle 5 yıllık sağ kalım oranları %40 oranında
kalmaktadır. Ubiquitinasyon; DNA yıkımının onarımında hücre döngüsünün
düzenlenmesinde rol alan temel düzenleyici işlemdir. Pek çok çalışma
ubiquitinasyon ve tümörogenez arasındaki bağlantıyı ortaya çıkarmaya
çalışmaktadır. Ubiquitin Spesifik Proteaz 17 (USP17); RAS yolağı üzerinden
sinyal göndererek hücre içi RAS lokalizasyonlarını, küçük GTPaz’lar ve hücre
proliferasyon ve migrasyonunu kontrol eder. Tumor protein translationally
controlled 1 (Tpt1), translationally controlled tumor protein (TCTP) olarak da
bilinen, bitkilerden memelilere kadar pek çok canlıda benzer yapıda bulunan bir
proteindir. Tpt1 kanser progresyonu ile ilişkilidir ve birçok tümörde yüksek
miktarlarda eksprese edilmektedir. Tpt1 in son zamanlarda p53 ü negatif olarak
etkilediği ve bozunumunu artırdığı ortaya kondu.
GEREÇ-YÖNTEM:
Araştırma Haziran 2013- Haziran2017 yıllarında arasında adneksiyal kitle
nedeniyle opere olmuş ve kesin patoloji sonuçları yüksek dereceli seröz over
kanseri olarak saptanan hastaların patoloji blokları üzerinde USP17 ve TPT1
reseptör kitleriyle yapılacak boyama sonuçları incelendi. Bu hastaların primer
doku ve metastatik doku piyeslerindeki reseptör pozitiflik oranı araştırıldı.
Reseptör pozitifliğinin metastaz ile ilişkisi araştırılması planlandı.
TARTIŞMA:
Daha önceden yapılmış olan
çalışmalarda farklı kanser tiplerinde
değerlendirilmiş olan USP17 ve Tpt 1 ekspresyonları yüksek dereceli seröz over kanseri olan hastaların primer ve metastatik tümör dokularında değerlendirildi ve her
iki grupta da
ekspresyonlarda artış izlendi.Bu
artışın tümör progresyon ve metastazı ile
ilişkili olduğu üşünülmekle birlikte
hasta sayılarının düşük olması sebebiyle istatistiksel olarak anlamlı
fark elde edilmedi.
SONUÇLAR: USP 17’nin primer ve metastatik dokudaki ekspresyonları
arasında USP 17 ile ilgili
immunhistokimyasal ekspresyonlar ile primer
over dokusu ve metastatik dokuya
ait ekspresyonlar arasında istatistiksel
olarak anlamlı bir fark elde edildi p=0,047 korelasyon katsayısı 0,349 olarak
zayıf da olsa bir korelasyon bulunmasına
rağmen Tpt 1 ile ilgili ekspresyonlarda
istatistiksel anlamlı bir korelasyon saptanmadı.

OBJECTIVE:
Ovarian cancer is the second most common gynecologic malignancy and the most
common cause of gynecologic cancer death. High-grade serous ovarian cancer
(HGSC), accounting for approximately 70% of epithelial ovarian canser, is an aggressive
ovarian cancer that associated with a worse clinical outcome. It is a
poor-prognosis tumor with a 5-year survival rate of around 40%, which is due to
late diagnosis (75% in FIGO III/IV) as well as the development of resistance to
chemotherapy. Ubiquitination is widely recognised as a fundamental regulator of
numerous cellular events, indeed the role of ubiquitination in DNA damage
response, cell cycle regulation and intracellular trafficking has been
extensively characterised. Consequently, much attention has been focused on
determining the association between ubiquitination and tumorigenesis. Ubiquitin
Specific Protease 17 (USP17) and found that it regulated signalling through the
Ras pathway by controlling the intracellular localisation of Ras and other
small GTPases; key regulators of cell proliferation and migration. Tumor
protein translationally controlled 1(Tpt1), also known as translationally
controlled tumor protein (TCTP), is an evolutionary conserved protein that
shares a high degree of homology from plants to mammals, implicating its
fundamental functions. Tpt1 is associated with cancer progression and is highly
expressed in many tumors. Recently, Tpt1 was found to interact with p53 and to
negatively regulate p53 by increasing its degradation.
MATERIAL-METHOD: The study examined the results of staining with USP17 and
TPT1 receptor kits on pathologic blocks of patients diagnosed with high-grade
serous ovarian carcinoma due to who had operation because of adnexal mass
between June 2013 and June 2017 and definitive pathology results. The rate of
receptor positivity of these patients in primary tissue and metastatic tissue
was investigated. It was planned to investigate the relationship between
receptor positivity and metastasis.
DISCUSSION:
Previous studies of USP17 and Tpt1 expressions evaluated in different types of
cancer, we evaluated in primary and metastatic tumor tissues of patients with
high grade serous over cancer and increased expression was observed in both
groups. Although the increase was considered to be associated with tumor
progression and metastasis statistically significant difference was not
obtained due to low number of patients.
CONCLUSIONS: There was a statistically significant difference between
primer and metastatic tissue expressions of USP 17 and USP 17-related
immunohistochemical stainings and expression of primer over tissue and
metastatic tissue. The correlation coefficient 0.349 p = 0.047 , although there
was a correlation it was weak, but Tpt 1 immunohistochemical staining showed no
statistically significant correlation.
