An Unusual Complication of Zygomaticomaxillary Complex Fracture: Fistula From the Maxillary Sinus To The Lateral Orbital Rim

Özet

In zygomaticomaxillary complex(ZMC) fractures treated with miniplates and screws, early postoperative complications are relatively common, whereas late complications are considered rare. This case report presents a 67-year-old male who developed a cutaneous fistula 18 years after ZMC fracture repair. Imaging revealed a fistulous tract extending from the lateral orbital rim to the maxillary sinus. As a result of the surgical exploration, a malpositioned silicone implant most likely inserted during the initial surgical intervention was identified. The fistula and foreign body were excised, and the defect was reconstructed using buccal fat flap. This case highlights the potential for very late complications from retained foreign materials and underlines the importance of autologous tissue reconstruction.
