Opalski syndrome which is due to a lesion of the lower medulla involving the corticospinal tract after the pyramidal decussation, results from an occlusion of the vertebral artery and in this syndrome ipsilateral hemiparesia is associated with symptoms of lateral medullary syndrome (Wallenberg syndrome). In this rarely seen syndrome, respiratory insufficiency can also be seen in a few patients in addition to classical clinical findings. In this manuscript, a case with Opalski syndrome who has respiratory insufficiency is presented.
