Amaç:
Mukormikoz, anjiyo-invaziv fırsatçı bir enfeksiyondur ve genellikle nazal ve
paranazal sinüslerde başlar. Enfeksiyon özellikle organ transplantasyonu
yapılmış hastalarda, tedavideki gelişmelere rağmen, yaşamı tehdit eden ciddi
problemlere sebep olabilmektedir. Rino-orbito-serebral
formu, orofasiyal bölgede ortaya çıkabileceğinden, diş hekimleri için özellikle
önemlidir. Mukormikoz, damar endotelini etkileyerek vasküler yetmezliğe ve
mukormikoz osteomiyelit ile sonuçlanan kemik nekrozuna neden olabilmektedir. Bu
vaka raporunda, renal transplant yapılmış invaziv fungal sinüzit tanılı hastanın
ağız bulguları multi-disipliner olarak değerlendirilmiştir.
Olgu sunumu: Hipertansiyon
nedeniyle renal yetmezlik gelişen, üç yıl önce renal transplantasyonu yapılmış
38 yaşındaki erkek hasta, iki ay önce üniversitemiz Kulak Burun ve Boğaz (KBB) Hastalıkları
Anabilim Dalında endoskopik sinüs cerrahisi ve medial orbital dekompresyon
uygulanarak histopatolojik olarak invaziv fungal sinüzitis tanısı almıştır. Hastanın
extra-oral muayenesinde sol gözünde preoperatif dönemden itibaren Frozen-eye
mevcuttu. Hastaya, 20 gün IV Amfoterisin B tedavisi uygulanmıştır. Hastanın,
maksiller bölge dişlerinde klinik olarak gözlenen mobilite sebebiyle Periodontoloji
Anabilim Dalına konsülte edilmiştir. İntra-oral muayenesinde, maksillada expose
kemik alanları ve tüm maksillayı içeren alveolar segmental mobilite mevcuttu. Hastanın
klinik periodontal değerlendirmesi yapıldı, hastanın Evre III-Derece C, lokalize
periodontitis olduğu saptandı ve hastaya birinci basamak periodontal tedavi
uygulandı (diş yüzeyi temizliği ve oral hijyen eğitimi). KBB tarafından
mukormikoz intra-kranial tutulumu mevcut olduğu bildirilen hastaya ek cerrahi
debridman önerilmedi.
Sonuç: Hekimler organ transplantasyonu gibi risk faktörlerine sahip olan
hastalarda, mukormikoz açısından şüpheci olmalı ve şüphe duyulan olgularda KBB’ye
gecikmeden konsülte etmelidir. Diş hekimleri, bu tür hasta
gruplarında kapsamlı intra ve ekstra oral muayene için disiplinler arası
ekiplere dahil edilmelidir. Böylece henüz başlangıç aşamasındaki ağız içi
lezyonların zamanında tespiti yapılarak, koruyucu yaklaşımlar uygulanabilir ya
da mevcut enfeksiyonların ilerlemesi doğru tedavilerle kontrol altına alınarak,
mortalite riski azaltılabilir.
Aim:
Mucormycosis is an angio-invasive
infection and usually starts in the nasal and paranasal sinuses.
Infection can cause life-threatening events despite advances in treatments,
especially in transplant patients. The rhino-orbito-cerebral form is of particular
importance to the dentists as it can be observed in the orofacial region. Mucormycosis
can affect the endothelial layer of blood vessels, causing vascular
insufficiency and bone necrosis, resulting in mucormycosis osteomyelitis. Present
case report evaluated oral findings of a patient with invasive fungal sinusitis
who underwent renal transplantation.
Case Report:
The case report concerns a 38-year-old male patient who developed renal failure
due to hypertension and had a renal transplant three years ago. Invasive fungal
sinusitis was diagnosed histopathologically two months ago, as the result of
endoscopic sinus surgery and medial orbital decompression performed by the
Department of Otorhinolaryngology (ENT). Extra-oral preoperative examination was
confirmed as “frozen-eye” in the left eye. The patient underwent Amphotericin B
IV treatment for 20 days. The patient was referred to the Department of
Periodontology due to teeth mobility in the maxillary teeth. Exposed bone areas
and alveolar segmental mobility in the entire maxilla were detected clinically.
Periodontal examination was performed and the patient was diagnosed as localized
Stage III, Grade C periodontitis. First-step periodontal treatment (scaling&oral
hygiene instruction) was performed. Due to intra-cranial involvement further
surgical debridement was not recommended by the ENT.
Conclusion: Physicians should be aware of mucormycosis in
patients having risk factors especially organ transplantion. Referral to the ENT should considered without any delay in
suspected cases. Dentists should
be a part of interdisciplinary team for comprehensive intra&extra oral
examination. Thus, early detection of intraoral lesions can be made timely,
preventive approaches can be applied or the progression of existing infections
may be controlled with the targeted treatments. In this way the risk of
mortality could be reduced/avoided.
