Severe secondary hyperparathyroidism has been reported to develop in children with chronic renal failure treated with recombinant human growth hormone due to growth retardation. We report here a 15-year-old-boy undergoing a chronic hemodialysis program who developed severe secondary hyperparathyroidism within nine months of recombinant human growth hormone therapy. His alkaline phosphatase and parathormone levels increased progressively during therapy (2749 IU/L, 425 pg/ml, respectively). After discontinuation of growth hormone treatment at 12th month due to insufficient height velocity, and administration of high dose calcitriol, his alkaline phosphatase and parathormore levels decreased within four months.
