Amaç: Articulatio atlantooccipitalis
anatomisinin detaylı bilinmesi, craniovertebral bileşkenin anomalileri ve
cerrahi tedavilerinde önem taşır. Atlas’ın facies articularis
superior’unun (FAS) morfolojisindeki değişiklikler, aticulatio
atlantooccipitalis 'de instabiliteye yol açarak hareketlerin kısıtlanmasına veya
hipermotiliteye yol açarak vasküler ve nörolojik semptomlara neden olabilir.
Çalışmada eklemi oluşturan yüzeylerden biri olan atlas’ın FAS’ının morfolojik ve morfometrik incelenmesi
amaçlanmıştır.
Gereç ve Yöntemler: Dokuz Eylül Üniversitesi Tıp Fakültesi Anatomi Anabilim
Dalı Laboratuvarı’na ait kemik
koleksiyonundan, yaşı ve cinsiyeti
bilinmeyen 70 adet atlas’ta gerçekleştirildi. Makroskopik inceleme ve
digital kaliper kullanılarak ölçümler yapıldı.
Bulgular: FAS’ın anteroposterior
uzunluğu sağda 23.08 ± 2.61 mm, solda 23.29 ± 2.54 mm, ortalama 23.19 ± 2.57 mm
ölçüldü. FAS’ın transvers uzunluğu sağda 11.06
± 1.20 mm, solda 11.09 ± 1.03 mm, ortalama 11.07 ± 1.11 mm ölçüldü. FAS yüzey
şekline göre incelendiğinde 50 ( %39.1) adet sekiz, 29 (%22.7) adet böbrek, 19
(%14.8) adet oval şeklinde olduğu gözlendi.
Sonuç: Craniovertebral eklem
insitabilitesinin uygun ve başarılı cerrahi tedavisi, FAS anatomisinin
detaylı bir şekilde bilinmesini gerektirir. Bu çalışmada bulunduğumuz
coğrafyaya ait verilerin elde edilmesi ve cerrahi tedavilerde bulunan sonuçlardan yaralanılmasını umuyoruz.

Objective: Understanding the detailed anatomy of the atlantooccipital
joint is crucial in diagnosis and surgical treatment of
craniovertebral junction anomalies. Changes in the morphology
of the superior articular facet (SAF) of the atlas, one of the
surfaces that forms the joint, can lead to instability, restricted
movement, hypermobility, and vascular and neurological
symptoms. This study aims to investigate the morphology and
morphometry of SAF of the atlas, which is one of the surfaces
that forms the joint.
Methods: Seventy atlases from the bone collection of
Department of Anatomy, Faculty of Medicine, Dokuz Eylül
University, with unknown age and gender, were examined.
Measurements were taken using digital caliper and macroscopic
examination and digital calipers.
Results: The anteroposterior length of the SAF was measured
as 23.08±2.61 mm on the right side and 23.29±2.54 mm on
the left side. The transverse length of SAF was measured as
11.06±1.20 mm on the right side and 11.09±1.03 mm on the
left side. The shape of the SAFs were evaluated macroscopically
and classified in three types. The types and frequencies
were as followinfg: 50 (39.1%) were eight-shaped, 29 (22.7%)
were kidney-shaped, and 19 (14.8%) were oval-shaped.
Conclusion: Successful surgical treatment of craniovertebral
joint requires a detailed understanding of the anatomy of the
atlas. The data obtained from our study is important in terms
of our large number of bone collection from a spesific geographic
region and may be helpful for clinicians, who perform
surgical treatments of craniovertebral joint.
