Keratosis follicularis spinulosa decalvans (KFSD) is a genetically and clinically heterogeneous dermatosis mainly characterized by extensive keratosis pilaris of the trunk and extremities, follicular inflammation and progressive cicatricial alopecia of te scalp, eyebrows and eyelashes. Less frequent associations are the development of scalp pustules, palmoplantar keratoderma and ocular abnormalities. Although there are conflicting results for efficacy of treatments in KFSD, generally partial, transient or unsuccessful response rates have been reported with various treatment methods including topical emollients and keratolytics, topical and intralesional corticosteroids, systemic antibacterials, oral etretinate, isotretinoin and dapsone. Herein, we report a 21 year-old-male of KFSD who responded partially to acitretin treatment with resolution of the follicular hyperkeratosis and pustules of the scalp.
