Özet

Aim: Soave operation can be performed in single stage entirely through transanal approach without a preliminary colostomy. Herein we report our experience with this operation. Patients and Method: Transanal Soave operation was performed in three patients with Hirschprung disease, ages varying from one month to one year. The colon proximal to peritoneal reflection could be easily inverted after mucosal stripping. Aganlionic segment could be easily pulled through rectal muscular cuff and its mesenterium could be divided with same ease. Transition zone was located in the rectosigmoid region in all patients; thus entire operation could be performed transanal approach. Results: Oral feeding was started on the 1st postoperative day and was tolerated well. Intraoperative blood loss was minimal and none of the patients required blood transfusion. Postoperative analgesic requirements were minimal. All were discharged within 4 days. Their one year follow up was uneventful. Conclusion: In patients with Hirschprung's disease having transition zones in rectosigmoid region, single stage Soave operation performed entirely through transanal approach without a preliminary colostomy seems to have many advantages. Short recovery period is highly convenient for both patients and parents. Short hospital stay reduces treatment costs significantly. Since aganglionic segment is located in the rectosigmoid region in 75 % of patients with Hirschsprung's disease, a significant portion of them might benefit from one stage Soave operation performed entirely through transanal approach.
