Özet

Objective: Circulating microRNAs (miRNAs) are promising non-invasive biomarkers for prognosis and recurrence prediction. We evaluated whether serial circulating miRNAs predict recurrence and survival in resected NSCLC.

Methods: Prospective cohort of 34 NSCLC patients undergoing curative surgery (lobectomy) between 2021–2023. Blood samples were obtained preoperatively, at 1. month postoperatively, and at 1 year postoperatively (or at recurrence). Expression of seven miRNAs (miRNA-21, -103, -146a, -200b, -203, -221, -222) was measured by RT-PCR. Associations between miRNA changes and recurrence/DFS were assessed using Kaplan–Meier and Cox regression.

Results: Of 34 patients (88.2% early stage), 58.8% developed recurrence; mean DFS was 23 months. Rising circulating miRNA-146a at 1 year (or at relapse) was strongly associated with recurrence (p=0.003). In Cox analysis, patients with higher miRNA-146a had a 5.07-fold higher recurrence risk (HR 5.07; 95% CI 1.73–14.89; p=0.003), whereas lower miRNA-146a was associated with longer DFS (34 months).

Conclusion: Serial miRNA-146a increasing independently predicts recurrence after NSCLC resection, supporting miRNA-146a as a candidate biomarker for postoperative risk stratification—particularly relevant given high recurrence even in early-stage disease.
