Özet
Introduction
A comprehensive approach focuses on achieving clinical health while ensuring the biological and functional stability of the entire oral environment. In complex cases where natural teeth and dental implants coexist, this approach aims to optimize long-term survival by integrating regenerative materials and modern decontamination techniques.
Case Description
A systemically healthy 53-year-old female diagnosed with Stage 3 Grade B periodontitis was treated. Following Phase 1 therapy, flap surgery with Platelet-Rich Fibrin (PRF) was performed on infrabony periodontal defects at sites 11 and 24. An implant was placed at site 14 to restore masticatory function. For peri-implantitis at site 36, surgical access was followed by surface decontamination using a titanium brush (Ti-brush) and PRF stabilization. The patient was monitored for approximately 3 years, comprising an 18-month active treatment phase and a 20-month supportive periodontal therapy period. At the 3-year follow-up, periodontal health was established, significant clinical attachment was gained, and functional stability was maintained.
Discussion
Long-term follow-up data validate the success of this holistic protocol. While PRF enhances regenerative potential in infrabony periodontal defects, the Ti-brush ensures effective decontamination without compromising implant surface integrity. Stability during the maintenance phase underscores the critical role of regular professional follow-up and patient compliance in the success of regenerative therapies.
Conclusion/Clinical Significance
Simultaneous management of periodontal and peri-implant pathologies provides predictable outcomes in complex cases. This 3-year follow-up demonstrates that the combination of PRF and mechanical decontamination tools is a reliable methodology for ensuring long-term oral health.
