Yumuşak ve sert dokuların
tamir ve rejenerasyonunda çok sayıda greft materyali, biyolojik maddeler ve
membranlar kullanılmaktadır. Son dönemde yaygınlaşan doku mühendisliği
uygulamaları rejeneratif tedavilerde yeni bir dönem başlatmıştır. Buna rağmen,
kanıta dayalı bilimsel literatürde değişmeyen gerçeklerden biri de otojen
materyallerin günümüzde de altın standart olduklarıdır. Son on yılda otojen kan
konsantrelerinin kullanımı tıp ve dişhekimliği alanlarında önem kazanmıştır. Büyüme
faktörleri, lökositler açısından zengin ve otojen bir biyomateryal olan
trombositten zengin fibrinin (PRF), klinik vakalarda doku iyileşmesini
hızlandırdığı gösterilmiştir. PRF’ nin kısa sürede hazırlanması, içerisine
dışardan herhangi bir antikoagülan materyal ilave edilmemesi, birçok hücresel
yapının özellikle lökositlerin, büyüme faktörleriyle birlikte fibrin matrix
içerisinde yerleşmesi sonucunda, PRF doku rejenerasyonunda ve yara
iyileşmesinde çok değerli bir materyal haline gelmiştir. PRF’ nin en güncel kullanım
yöntemlerinden biri enjekte edilebilen (i-PRF) formudur. Bu uygulama, fibrine
dönüşmeden önce kısa süreli santrifüj uygulaması ile sıvı trombosit yapıda
öncelikle likit trombin ve fibrinojen içermesini gerektirir. Hidrofobik yüzeyli
plastik tüpler kullanıldığı için pıhtılaşma etkin bir şekilde başlamaz. Sonuç
olarak 3 dakikalık kısa bir santrifüjleme süresi ile kan kısımlarının tamamı
santrifüjleme etkisiyle tüpün üst kısmına ulaşır. i-PRF, plazma ve trombosit
karışımı olan açık sarı renkli tabakanın aspire edilmesiyle elde edilir. İ-PRF
ile yüksek konsantrasyonda elde edilen büyüme faktörleri ve sitokinler,
rejeneratif hücrelerde artış sağlayarak, sert ve yumuşak dokulardaki iyileşmeyi
hızlandırırlar. Bu derleme, i-PRF' nin iyileşme sürecindeki avantajlarına,
rejeneratif cerrahide ve ağız lezyonlarındaki güncel yaklaşımlarına
odaklanmaktadır.
Many graft materials,
biological materials and membranes are used in the repair and regeneration of
soft and hard tissues. Tissue engineering applications, which
have become widespread recently, have started a new era in regenerative
treatments. However, one of the unchanging facts in the
evidence-based scientific literature is that autogenous materials are still the
gold standard today. Over the last decade, the use of autologous blood
concentrates has gained importance in the medical and dental areas. Platelet-rich
fibrin (PRF) is an autogenous biomaterial rich in growth factors, leukocytes
that has been shown to accelerate tissue healing in clinical cases. As a result of the short preparation time of PRF, the
absence of any anticoagulant materials from the outside, the placement of many
cellular structures, especially leukocytes, together with growth factors in the
fibrin matrix, it has become a very valuable material for tissue regeneration
and wound healing. One of the most current methods of using PRF is its
injectable form. That procedure requires centrifugation for a short period to
generate liquid platelet concentration, principally involving liquid thrombin
and fibrinogen before forming fibrin. Since plastic tubes with a hydrophobic
surface are used, the coagulation does not begin efficiently. As a result, with
a short centrifugation period of 3 minutes, all of the blood portions reach the
tube's top under centrifugation power. i-PRF is obtained by aspirating the
light-yellow coloured layer, which is a mixture of plasma and platelets. Growth
factors and cytokines obtained in high concentrations with i-PRF accelerate the
healing of hard and soft tissues by increasing regenerative cells. This review
focuses on the advantages of i-PRF in the healing process, its current
approaches in regenerative surgery and oral lesions.
